Instructions:

  • In order to qualify as a counselor you need to be at least 18 years old by the time of the start of the program and be currently enrolled as a student in good standing in an undergraduate or graduate program at a college/university.
  • Print out and complete clearly this application form and mail it to us.
  • Either enclose with your application, or ask your college/university to send us an official transcript.
  • If you have never attended the SMaRT camp, ask at least one of your mathematics professors to mail, FAX, or e-mail directly to us a recommendation letter.
